Impact of an antifungal prophylaxis protocol in liver transplantation in Grenoble Alpes university hospital center

Description:
Fungal infections are a major cause of morbidity and mortality in orthotopic liver transplant patients (OLT). The aim of this monocentric before/after study was to assess the impact of the antifungal prophylaxis realized in Grenoble-Alpes University Hospital Center (GA-UHC), on the occurrence of fungal events and on survival in OLT patients with invasive fungal infections (IFls) risk factors. We retrospectively analyzed the clinical data of 97 liver transplant patients in GA-UHC, between the 1 st January 2015 and the 1st June 2023 (32 with protocol and 65 wilhout protocol). Results: 34.0% (33/97) of patients developed al least one IFI at 6 months, without statistically significant difference between both groups. ln survival analysis, the median time of onset of IFI was 10.5 days. Fifty-two of 59 events were considered as proven or probable IFI. Proven IFI were frequent, 55.0% (11/20) in protocol-group and 65.6% (21132) in no-protocol group (p-value=0.443). Recorded IFI were mainly ruptured or unruptured hepatic yeast pseudoaneurysm (18.6%; 11/59) and positive cultures from surgical drain (37.3%; 22/59), in both groups. Most IFIs were monomicrobial and most fungal agents were Candida albicans (73.0%, 27/37) and Candida glabrata (18.9%, 7/37). Analysis of antifungals showed no evidence of the emergence of resistance in protocol group. This study confirms the unfavorable outcome of patients presenting an invasive fungal infection alter OLT: there were more deaths at 6 months in patients with IFIs, 30.3% (10/33) vs 10.9% (7164) without IFI (p-value=0.017). We also found that overall mortality was significantly higher in patients at risk of IFIs who did not benefit of an antifungal systematic prophylaxis with caspofungin: 41.5% (27/65) patients died in no-protocol group vs 15.6% (5/32) in protocol group (p-value=0,02). But the number of deaths was not significantly different tl 6 months (pvalue=0.166) and 42 months (p-value=0.188). ln survival analysis, median survival time for death was 172 days in protocol group and 159 days in no-protocol group, p-value = 0.64 (Log-Rank test). This study showed the caspofungin protocol did not seem ta have any impact on the occurrence of IFIs or on survival among liver transplanted patients. Another multi-centric study with a longer follow up to compare the occurrence of IFis and the minimum effective time protocol would be interesting for standardizing practices. Les infections fongiques constituent une cause majeure de morbidité et mortalité chez les patients transplantés hépatiques (TH). Cette étude avant/après, monocentrique, a pour but d’étudier I' impact de la prophylaxie antifongique réalisée au CHUGA sur I'incidence des complications fongiques et la mortalité des patients transplantés à risque d’infection fongique invasive (IFI). Méthodes : Nous avons analysés rétrospectivement les données cliniques de 97 patients transplantés hépatiques et hospitalisés au CHU de Grenoble-Alpes entre le 1er janvier 2015 et le 1er juin 2023 (32 avec protocole et 65 sans protocole). 34,0 % (33/97) des patients ont eu au moins une IFI à 6 mois, sans différence statistiquement significative entre les deux groupes. Dans l'analyse de survie, le délai médian d'apparition de l'IFI était de 10.5 jours. Cinquante-deux des 59 événements relevés ont été considérés comme des IFI prouvées ou probables. Les IFI prouvées étaient fréquentes, 55,0 % (11 /20) dans le groupe protocole et 65,6 % (21/32) dans le groupe sans protocole (p=0,443). Les IFI étaient principalement des pseudo-anévrismes hépatiques à levure, rompus ou non rompus (18,6 % ; 11/59) et des infections de liquide de lame (37,3 % ; 22/59), dans les deux groupes. La plupart des IFI étaient mono-microbiennes et les principaux agents fongiques étaient Candida albicans (73,0 %, 27/37) et Candida glabrata (18,9 %, 7/37). L'analyse des antifongigrammes n'a montré aucun signe d'émergence de résistance dans le groupe protocole. Cette étude confirme l'évolution défavorable des patients présentant une infection fongique invasive post-TH : le nombre de décès à 6 mois chez les patients ayant une IFI était de 30,3% (10/33) vs 10,9% (7/64) sans IFI (p= 0,017). Nous avons également constaté que la mortalité globale était significativement plus élevée chez tes patients à risque d'IFI n'ayant pas bénéficié d'une prophylaxie antifongique par caspofungine : 41.5 % (27/65) des patients sont décédés dans le groupe sans protocole vs 15,6 % (5132) dans le groupe protocole (p =0.02). Cependant le nombre de décès à 6 mois (p-value=0.166) et à 42 mois (p-value=0.188) n'était pas significativement différent. Dans l'analyse de survie, la durée médiane de survie était de 172 jours dans le groupe protocole et de 159 jours dans le groupe sans protocole, (p = 0,84) (test Log-Rank). Notre étude n'a pas permis de mettre en évidence un impact du protocole antifongique sur I'incidence des IFI ou sur la survie des patients transplantés hépatiques à risque. Une nouvelle étude multicentrique, avec une durée de suivi plus longue, serait intéressante pour comparer les délais de survenue d'IFI et définir la durée minimale de traitement nécessaire afin de standardiser les pratiques.
