Background: Weight gain in adults is associated with an increased risk of obesity related diseases and high health care costs. However, there are limited studies on weight gain in Asians. This study investigated the prevalence of comorbidities and factors associated with weight gain in Korean adults. Methods: This is a case-controlled study using a nationally representative sample of Korean population. We analyzed data of 11,477 adults who participated in the Korea National Health and Nutritional Examination Survey 2016-2019. Weight change was assessed using a self-reported questionnaire, and the amount of weight gain was categorized into 3-6 kg, 6-10 kg, and > 10 kg groups. We calculated odds ratios (ORs) and 95% confidence intervals (CIs) for weight gain according to associated factors through a multivariable logistic regression analysis. Results: The overall prevalence of weight gain was 25.7% in men and 31.3% in women. According to amount of weight gain, the prevalence was 20.1% and 26.7% for 3-6 kg, 7.1% and 6.6% for 6-10 kg, and 3.4% and 3.2% for > 10 kg among men and women, respectively. The ORs for weight gain increased significantly in a graded fashion with age in younger participants in both sexes. Furthermore, severe weight gain (>10 kg) was more prevalent with a higher baseline weight. The factors associated with weight gain were being unmarried, blue collar job, lower income, and alcohol drinking in men; being married in women and; smoking and skipping breakfast in both sexes. Conclusions: Weight gain was much more prominent in younger adults and at higher baseline weight in both sexes. Public education to prevent weight gain should be reinforced by considering its associated detrimental factors among Korean adults.
