  • Description: 다발성 경화증과 관련된 시신경염의 시력 예후는 인종에 따라 다른 것으로 알려져 있으나,국내에서는 이 시력 예후에 대하여 보고된 바 없다.이에 저자들은 1992년부터 1998년 사이에 다발성 경화증과 관련된 시신경염으로 진단된 환자의 시력 예후를 후향적으로 조사하였다. 전체 23명의 38안에 대하여 평균 1 6개월 동안 추적 관찰하였다. 23명 중 14명은 임상적으로 진단된 다발성 경화증(clinically diagnosed multiple sclerosis)이었으며 9명은 검사상 진단된 다발성 경화증(laboratory supported multiple sclerosis)이었다. 임상적으로 진단된 환자군에서 검사상 진단된 환자군보다 시신경염이 재발하는 경향을 보였으며(3.1:2.6),최종 내원시력이 0.5 이상으로 회복되는 빈도가 낮았다(26%: 60%). 한국인에서 다발성 경화증과 관련된 시신경염은 재발하는 경향이 있으며, 시력 예후가 불량하였다. 이러한 결과는 한국인 다발성 경화증과 관련된 시신경염에 대한 경과를 예측하는 데 도움을 줄 수 있을 것으로 생각한다(한안지 41:1216~1223, 2000). There have been few reports on the visual prognosis of optic neuritis(ON) associated with multiple sclerosis(MS) in Asians, although previous reports have suggested a racial effect on visual prognosis in ON associated with MS. Therefore, in this study we focused on the assessment of visual prognosis in ON associated with MS in Koreans between 1992 and 1998. Thirty-eight eyes from 23 patients were enrolled and the mean follow-up was 16 months. Of the 23 patients, 14 patients had clinically diagnosed MS(CMS) and 9 patients had laboratory supported MS(LSMS). The patients with CMS tended towards more recurrent ON than those with LSMS(3.1:2.6) and relatively low proportion of CMS patients had 0.5 or better final visual acuity(26%:60%). Korean patients with ON associated with MS were at greater risk of recurrence, and visual prognosis was poor. These results have important clinical implications for the assessment of ON associated with MS in Koreans(J Korean Ophthalmol Soc 41:1216~1223, 2000).
